On December 23, 2025, the Centers for Medicare & Medicaid Services (CMS) announced the availability of a new training resource focused on the data validation process for skilled nursing facility (SNF) assessment-based measures. The resource is intended to support SNFs participating in the SNF Value-Based Purchasing (VBP) Program and the SNF Quality Reporting Program (QRP).
CMS is offering a recorded presentation hosted on YouTube that provides an overview of the data validation process, including its purpose and structure. The training outlines how SNFs will be notified of selection, the medical record submission process, required activities for selected facilities, and where to locate key program resources. The presentation is designed to help facilities prepare for participation in the validation process and better understand CMS expectations.
In addition to the recorded presentation, CMS has made the presentation slides and a full transcript available for download, along with a list of acronyms and an updated set of Frequently Asked Questions (FAQs) dated December 17, 2025. Links to these resources are available here.
Compliance Perspective
Issue
CMS has released additional guidance and training related to the data validation process for SNF assessment-based measures used in the SNF Value-Based Purchasing (VBP) Program and the SNF Quality Reporting Program (QRP). As CMS continues to implement and operationalize the data validation process, SNFs must be prepared to respond to validation activities, including the submission of complete and accurate medical record documentation within required timeframes. Facilities that do not maintain effective oversight, clearly defined internal processes, and ongoing readiness may be at increased risk for noncompliance findings and associated program or payment impacts.
Discussion Points
- Facilities should review and update policies and procedures related to the Minimum Data Set (MDS) completion, medical record documentation, and regulatory monitoring to ensure alignment with current CMS data validation expectations. Policies should clearly define responsibilities for monitoring CMS communications, compiling requested documentation, and meeting required timelines. Facilities may benefit from collaborating with a consultant to review existing workflows, identify vulnerabilities, and ensure procedures support consistent documentation practices and regulatory readiness.
- Provide education to staff involved in MDS completion, documentation, and compliance oversight to reinforce understanding of assessment-based measure requirements and the importance of consistency between MDS data and the supporting medical record. Med-Net Academy offers the course Long-Term Care Survey Process, which covers entrance requirements, sample selection, survey phases, mandatory and triggered facility tasks, and the purpose and scope of the Survey Pathways. The course provides practical guidance to support staff readiness for regulatory oversight and compliance activities.
- Conduct periodic audits to evaluate readiness for CMS data validation activities, including reviews of MDS accuracy, supporting documentation, and internal tracking processes. Audits can help identify gaps, inconsistencies, or systemic issues before they result in compliance findings. Facilities may consider working with an external reviewer or consultant to perform focused or mock audits, providing an objective assessment and actionable feedback to strengthen compliance efforts.
